On Line Reporting System

Both the ‘Injured Person Report Form’ and ‘Violence to Staff Incident Report’ are only required when a colleague has not input their own incident onto the on-line reporting system.

Please note that without this signed document for incidents where a colleague did not enter the incident onto the on-line reporting system, the manager will not be able to complete the manager’s investigation.

This will also mean that the manager will not be able to close the incident down as this signed document is a specific requirement and must be uploaded onto the on-line system
